Doctors at Fortis Hospital saved the life of a 45-year-old man who arrived at the emergency department with sudden left-sided weakness, slurred speech and confusion after symptoms began about three and a half hours earlier. An urgent CT scan showed acute ischemic changes in the right Internal Carotid Artery (ICA) territory with no bleeding, and further imaging revealed a complete blockage in a major brain artery plus another critical blockage and a tear in the artery wall-an uncommon combination seen in fewer than 0.5% of stroke cases, said Dr Manik Mahajan, Additional Director (Neurology).
Because such presentations can be fatal in roughly 50–69% of patients if not treated promptly, the team started intravenous clot-dissolving medication immediately, following international stroke guidelines. The patient was then taken for an emergency endovascular procedure performed by the neuro-intervention team led by Dr Trimaan Singh Sikand. During the procedure, physicians removed the clots and placed a small stent to repair the torn artery and restore blood flow to the brain. The intervention was completed within an hour under local anaesthesia.
The combined use of timely intravenous thrombolysis followed by endovascular clot removal and stenting was credited with preventing death or permanent disability in this rare and high-risk case. The doctors’ rapid diagnosis and intervention highlight the critical importance of recognising stroke symptoms early and seeking emergency care without delay.
