Urgent Alert: High Vector Indices Signal Dengue Risk in Thiruvananthapuram

Vector indices are on the rise in several areas of the district, raising concerns about a potential increase in dengue cases. The district, often labeled as the state’s dengue capital, is experiencing a notable increase in mosquito density, which health officials warn could lead to a spike in dengue infections at any time. Currently, dengue is reported frequently in urban slums and coastal regions, along with sporadic cases noted in various urban wards.

As of now, the state has documented 10,499 confirmed cases of dengue and 28,380 probable cases, resulting in 51 confirmed deaths and 23 probable fatalities. Health officials indicate that, compared to previous years, 2025 is faring better, having no significant seasonal surge and maintaining relatively low case numbers. In contrast, 2024 saw 20,674 confirmed cases and 53,536 probable cases, with confirmed deaths totaling 128.

Historically, the region faced its worst dengue epidemic in 2017, when there were 21,993 confirmed cases and 66,329 probable cases, alongside 165 confirmed deaths. This outbreak overwhelmed hospitals, particularly in southern districts such as Thiruvananthapuram, marking it as the most severe since the virus’s introduction in 1997.

Dengue fever has become a persistent public health challenge in Kerala, characterized by cyclical outbreaks and seasonal spikes tied to monsoon patterns. The state is hyperendemic for all four dengue virus serotypes (DENV1–4), which circulate year-round, but explosive epidemics tend to occur approximately every four years. This situation is exacerbated by urbanization, climate change, population mobility, and increases in vector indices, as stagnant water forms breeding sites for Aedes mosquitoes.

While many dengue infections are mild or asymptomatic, the growing number of cases and high seroprevalence-indicating prior exposure to the virus-pose unique challenges. A joint study conducted by the WHO and the state health department from 2017 to 2020 among schoolchildren aged 9-12 in Thiruvananthapuram, Kollam, and Kozhikode found a statewide seroprevalence of 30.9%, peaking at 46.9% in Thiruvananthapuram. Most children were unaware of their previous dengue infections, reflecting widespread silent exposure to the virus.

Dengue virus consists of four distinct serotypes. While infection with one serotype provides immunity to that specific one, it can leave individuals vulnerable to severe secondary infections from other serotypes, increasing the risk of complications like dengue shock syndrome. Until 2017, DENV 1 and DENV 2 were the dominant serotypes in Kerala. However, a study in 2022 indicated a shift, with DENV 3 and DENV 4 now prevalent.

Public health experts emphasize the necessity for primary care physicians to recognize serotype-specific clinical features of dengue, enabling early identification of infections and potentially preventing severe complications.
