Beyond Iterative Deployment: Rebuilding AI Safety as an Institutional Discipline

By Sanjeev Sarma

AI Safety Is an Architecture Problem-not an Ethics Layer

We often describe AI safety as a policy function: a review board, a model card, a set of principles, perhaps a non-binding pledge. Yet as AI shifts from answering prompts to acting through tools, data, and identities, safety can no longer live in a policy document. It must be enforced in the runtime architecture.

The decisive control is not a warning label. It is the authority and technical capability to slow deployment, isolate an action, and stop the system before an error becomes a disaster.

Culture Is a Control System

In an essay in The Atlantic, David Robinson, a longtime OpenAI employee who led safety-report writing for major launches, said he was resigning because the company’s “culture is broken.” His central argument is that trial-and-error deployment, increasingly capable systems, coarse alignment measures, and unresolved agent risks require redundancy, cautious planning, external assurance, and stronger incentives for safety. OpenAI separately said it is strengthening testing, monitoring, security, responsible behavior, and third-party evaluation.

My view is straightforward: this is not a choice between innovation and safety. Responsible scale is an architectural requirement.

Safety Must Move Left-and Stay Operational

From an enterprise architecture perspective, the central mistake is treating a model release as the end of engineering. Release actually begins a new control phase. Capability, autonomy, and access to consequential systems-not marketing calendars-should determine deployment gates.

Each agent needs an isolated identity, least-privilege permissions, constrained network access, and auditable actions. High-impact operations should trigger contextual human approval, while every deployment needs rapid rollback, a kill switch, and explicit blast-radius limits. Redundancy is not paranoia; it prevents one flawed assumption from becoming an enterprise-wide incident.

Evaluation must be continuous, not a pre-launch checkbox. Companies need adversarial testing, independent red teams, real-world monitoring, and metrics for near misses and unsafe behavior-not only benchmark scores. Human reviewers need time, authority, and escalation paths; otherwise, “human in the loop” becomes theatre.

The deeper issue is organizational design. If product velocity rewards shipping while safety teams can only document residual risk, even excellent guardrails will eventually be traded for growth. Frontier AI requires a funded independent safety function, explicit deployment thresholds, board-level visibility, and someone with real authority to stop a launch.

That is where external assurance becomes strategic. Voluntary commitments can raise expectations, but they cannot replace enforceable minimum standards. As agents move from chat interfaces into enterprise workflows, assessment must cover the entire sociotechnical system: permissions, data quality, model behavior, human oversight, vendor dependencies, and incident response. Otherwise, companies may certify a model while ignoring the environment in which it acts.

This accumulated safety debt often appears later as outages, penalties, lost trust, and costs that are almost impossible to reverse.

Relevance Without Reflexive Copying

For India, AI governance has direct relevance, but not because it should copy Silicon Valley’s roadmap. As AI moves into credit decisions, healthcare, public services, and governance, mistakes can operate at population scale. Uneven connectivity, multilingual users, legacy systems, and limited human review make locally grounded evaluation essential.

Public-sector AI should include explainability, appeal mechanisms, auditable logs, and human recourse. Frugal innovation must mean efficient use of resources, not a lower safety standard.

Questions Boards Should Ask

Evaluating frontier AI requires asking whether capability and autonomy thresholds are defined before deployment, and if every agent can be revoked, isolated, and audited in real time. Furthermore, leaders must determine if independent evidence influences release decisions, and identify who can stop a launch and whether they will use that authority. Finally, organizations must clarify if near misses trigger learning or only generate paperwork.

The next defining advantage in AI may not belong to whoever builds the smartest model, but to whoever builds the most trustworthy system around it. Intelligence without containment is not innovation; it is unmanaged risk.
