South Carolina’s measles outbreak is significantly impacting the United States’ health landscape, potentially jeopardizing the country’s status of having eliminated the disease. As of Tuesday, the state reported 88 new confirmed cases, pushing the total to 646 since the outbreak began in October. This surge comes just a year after a notable measles outbreak in Texas, which resulted in over 750 infections and two fatalities among unvaccinated children, propelling U.S. cases to a 34-year high.
In light of these alarming statistics, the Pan American Health Organization (PAHO) has scheduled an emergency meeting for April to evaluate the measles elimination status of Mexico and the U.S. According to PAHO, a country can only be deemed free from measles if it has no circulating outbreak for over a year. The U.S. achieved its elimination status in 2000 due to extensive vaccination efforts, but this status is now at risk if PAHO finds current public health measures inadequate to control the virus.
The elimination label might seem like mere semantics, yet losing it would signify a troubling resurgence of measles in the country. Last year, the Centers for Disease Control and Prevention (CDC) recorded 2,242 confirmed measles cases across 44 states. The Texas outbreak spread to three other states before being declared over in August, only for South Carolina and areas bordering Utah and Arizona to see new outbreaks shortly afterward.
As of January 13, the U.S. has documented 171 cases in nine states this year, predominantly fueled by South Carolina’s alarming numbers. On January 17, Clemson University confirmed its first case, and Anderson University also reported an infection. Currently, 538 individuals in South Carolina are under quarantine due to potential exposure in various schools and grocery locations.
Infectious disease expert Demetre Daskalakis, a former CDC official, emphasized the gravity of the situation: “We are nowhere close to where we would need to be to say this outbreak is over.” Before widespread vaccination became available in 1963, nearly every child contracted measles before age 15, leading to an annual toll of 400 to 500 American deaths. Unfortunately, vaccination rates have been declining in recent years, which is concerning.
Paul Offit, a pediatrician and director of the Vaccine Education Center at the Children’s Hospital of Philadelphia, remarked, “We eliminated the memory of measles, so people just aren’t scared of it.” This decline in fear is troubling, especially considering the virus’s high contagiousness and serious complications, such as encephalitis and pneumonia. The measles, mumps, and rubella (MMR) vaccine is highly effective, with a 97% efficacy rate after two doses.
Adding to the challenges posed by the outbreak, misinformation has emerged as a significant threat. During the Texas outbreak, Health and Human Services Secretary Robert F. Kennedy Jr. advocated for unverified remedies like vitamin A and cod liver oil, which led to increased reports of vitamin A toxicity in children. Local health departments are grappling with the spread of such misinformation. Chrissie Juliano, executive director of the Big Cities Health Coalition, stressed the urgency of vaccination: “The most important thing right now is for people to get vaccinated.”
As the situation evolves, the emphasis remains on protecting public health through awareness and adherence to vaccination recommendations, ensuring that the U.S. can maintain its hard-won status of measles elimination.
