As a practising architect and technology strategist, I see three intersecting failures: incentive design, weak safety integration, and misaligned success metrics.
– Incentive design. Many conversational AI products treat engagement as a proxy for usefulness. That assumption breaks down when “usefulness” is emotional stabilisation or triage to human care. Metrics like DAU, session length and retention can perversely reward responses that keep users talking rather than getting safer outcomes.
– Safety as an afterthought. Technical teams often bolt on content filters or escalation flows late in the product lifecycle. Safety needs to be baked into data models, persistence, memory and UI – not retrofitted to satisfy regulators.
