Concerns Arise Over Vaccine-Derived Polio Case in Meghalaya
The recent diagnosis of a polio case in a two-year-old from Meghalaya has raised alarms. Although the case stemmed from a vaccine-derived strain (originating from the weakened poliovirus used in oral polio vaccines, or OPV), health officials are advising vigilance. Despite this, India retains its polio-free status. The West Garo Hills district, where the case was detected, has heightened its alert level.
Dr. Narendra Singhla from CK Birla Hospital in Delhi clarified that vaccine-derived polio is a rare occurrence. This condition arises when the weakened virus in OPV mutates, regains virulence, and in very scarce instances, causes paralysis. This mutation typically occurs in areas with low vaccination coverage and insufficient sanitation, leading to potential outbreaks.
Understanding Vaccine-Derived Polio
The mutation process involves the live attenuated virus in the OPV undergoing changes. In rare situations, these changes result in a virus that resembles the wild type and can induce paralysis. Transmission is primarily a concern in communities with low vaccination rates where the mutated virus can spread among unvaccinated individuals.
Types of Vaccine-Derived Polio
- VDPV Types 1, 2, and 3: These correspond to the different wild poliovirus strains initially included in OPV.
- Circulating Vaccine-Derived Poliovirus (cVDPV): Occurs when the mutated virus spreads within a community.
- Ambiguous Vaccine-Derived Poliovirus (aVDPV): Used when the source of the virus is unclear.
Who is at Risk?
Individuals who haven’t completed their polio vaccination schedule, particularly in regions with low vaccination rates, are at risk. Dr. Singhla emphasizes the importance of adhering to the recommended vaccination doses.
Global Efforts to Combat Vaccine-Derived Polio
To tackle this challenge, high-income countries are shifting from OPV to the Inactivated Polio Vaccine (IPV), which uses killed virus particles and doesn’t carry the risk of causing vaccine-derived polio. Enhanced surveillance and ensuring high vaccination coverage in communities are also crucial strategies to prevent the virus from mutating and spreading.
