Urgent Alert: India’s Fatty Liver Epidemic Turns Ordinary Hepatitis A Infections Into Life-Threatening Risks!
With nearly one in three adults in India estimated to suffer from fatty liver disease, medical professionals are sounding alarms about the potential dangers posed by Hepatitis A. While commonly viewed as a mild infection, Hepatitis A can precipitate acute liver failure in individuals already grappling with liver conditions such as fatty liver disease and cirrhosis. This warning is particularly timely as World Liver Day approaches on April 19, amid a worrying increase in metabolic liver diseases across various age groups in India.
Hepatitis A, a viral infection transmitted through contaminated food and water or through poor hygiene practices, typically presents symptoms like fever, fatigue, nausea, abdominal pain, and jaundice. “Although it generally resolves without complications in healthy individuals, it can have severe repercussions for those with pre-existing liver issues,” explains Dr. Anurag Shrimal, Director of Liver Transplant at Gleneagles Hospital in Parel. He notes that current estimates suggest that 38.6% of the Indian population is affected by fatty liver disease, a figure that soars to 52.8% among high-risk groups, such as those with obesity and diabetes.
The prevalence of fatty liver disease, officially referred to as metabolic dysfunction-associated steatotic liver disease (MASLD), is alarming. Hospital-based studies indicate that roughly 40.8% of adults are affected, while community studies employing advanced diagnostic tools like FibroScan report rates exceeding 43%. Official estimates from the Ministry of Health and Family Welfare suggest that between 9% and 32% of the population is impacted, revealing significant regional disparities. Once largely confined to urban areas, the condition is now prevalent in rural areas as well, with rates varying dramatically-from about 9% in some rural communities to upwards of 50% among urban residents-reflecting widespread lifestyle changes.
Dr. Shrimal elaborates on the risk posed by Hepatitis A for those with fatty liver disease, noting that an acute infection exacerbates existing liver damage, resulting in severe inflammation and potentially acute-on-chronic liver failure. “Younger patients without cirrhosis may recover with prompt treatment, but those with early cirrhosis or serious liver damage might face irreversible liver dysfunction,” he warns, underscoring the urgent need for liver transplantation in such cases.
Concerns about Hepatitis A are often downplayed, particularly among adults with pre-existing liver conditions. Dr. Rohan Chaudhary, Associate Director of HPB Surgery and Liver Transplant at Gleneagles Hospitals, advises those experiencing symptoms like jaundice, fatigue, or loss of appetite-especially if they have known liver issues-to seek immediate medical help. “Fatty liver can remain asymptomatic, particularly in younger individuals, resulting in a misplaced sense of security. A seemingly healthy person may be at significant risk if they contract Hepatitis A,” he cautions.
The rising burden of liver disease poses both clinical and economic challenges for India. According to Care Health Insurance, liver disease-related claims have surged by 100% over the past three years, with a 5-10% annual increase in cases among younger individuals. Notably, cases in Tier 2 and Tier 3 cities are also rising by 10-15% each year. Manish Dodeja, Chief Operating Officer at Care Health Insurance, emphasizes the need for families to reassess their health coverage as liver disease becomes an increasingly financial strain on households.
India’s commitment to tackling fatty liver disease is evident through its integration into the National Programme for Prevention and Control of Non-Communicable Diseases. However, experts insist on the necessity of increased awareness and early detection. Dr. Shrimal stresses that preventive measures against Hepatitis A are straightforward, advocating for good hygiene practices and access to safe food and water. Vaccination, particularly for those already vulnerable due to liver conditions, is also highlighted as a highly effective defense against the virus.
